Q. Where are you located and what are your hours of operation?
A. We are located in the Sterling Crossings Plaza just North of Fifteen Mile Road, on the West side of Schoenherr. We are open on Mon., Wed., & Thurs. from 7:00a.m. to 7:00 p.m. and Tues. & Fri. from 7:00 a.m. to 4:00 p.m.
Q. Do I need a written prescription from my Doctor for physical therapy?
A. Yes. The State of Michigan requires a physician’s signed prescription for physical therapy before we can begin treatment. The state does allow therapists to perform an evaluation and provide options for treatment without a prescription. Your written prescription is good for 30 days.
Q. How long will it take to get an appointment?
A. In most cases we can schedule you within 24 to 48 hours. Call our office at (586) 978-7900 for scheduling information.
Q. What should I bring with me on my first visit?
A. Please bring your prescription (see above), your driver’s license (or other valid I.D.), insurance cards, and appropriate attire (see next answer).
Q. What should I wear?
A. You should wear comfortable clothing and athletic shoes. We do have shorts and gowns available, but some people feel more comfortable bringing their own change of clothes. We provide private changing areas for you. If your treatment involves your knee it is helpful to bring shorts. For female shoulder patients a tank top is beneficial to properly treat the shoulder.
Q. What should I expect at my first appointment?
A. You will meet with a licensed, board certified physical therapist. Your therapist will evaluate your condition and needs, and provide a treatment plan specifically for you. You’ll be treated by your therapist your first visit and every visit.
Q. What insurance plans do you accept?
A. C&G PT accepts a wide variety of insurance plans, including worker’s compensation and automobile injury claims. See our insurance page for a partial list of plans we accept. Call our office with your insurance information and we will check your benefits for you that day. That way you know what to expect before you start your therapy.
Q. What if I was injured on the job or in an auto accident?
A. C&G PT accepts both worker’s compensation claims and auto injury claims. Once you have filed a claim with your employer (for worker’s comp) or your auto insurance company (for auto injury accidents), we will contact the insurer for approval of physical therapy.
Q. How long will my appointments last?
A. Each patient’s treatment will vary depending on your diagnosis and the treatment plan. Generally you can expect to be here 45 minutes to an hour and a half.
